In the event of loss or theft of your card

A duplicate will be sent to you by post only and a charge of 10 euros will be made.
Send:
• The completed application form for a duplicate card (downloaded from the Internet)
• A photocopy of your ID card
• A cheque for €10 payable to “Culturespaces SA” to the following address: 
Musée Jacquemart-André - Carte Privilège - 158, boulevard Haussmann 75008 Paris

Your duplicate will be sent to you within 15 days.
